SUBJECT: Recommendation to award bid for continuous forms.
Bid No. 1376.
Introduction
o
This report concerns award of bid for continuous forms in the
total amount of $3,140.71, including applicable tax. This award
is to be charged to account 01 -100- 223 - 000 -411, Data Processing
Standard Forms, during fiscal year 1978 -79.
Background
In response to published Notice Inviting Bids to furnish water
bills, attendance reports, and blank perforated cards in quantities
as ordered, bids were received and publicly opened and read in the
office of the City Manager May 25, 1978. Proposal forms were mailed
to eighteen (18) potential suppliers and notices were advertised
in accordance with city charter and municipal code provisions. Five
(5) responsive bids were received as listed on the attached summary.
Moore Business Forms was the lowest bidder on all items.
Recommendation
It is recommended that the award be made to Moore Business Forms, as
the lowest and best bidder, in the total amount of $3,140.71, including
applicable tax.
