Introduction
ThlS report presents for Council review a revised Board/Commisslon
appllcatlon form and a resolutlon amending Resolutlon No. 4993(CCS)
(whlch sets forth procedures) to provide for public review of appli-
catlOns.

The Clty Councll, at ltS meeting of August 23, 1983, dlrected the
Clty Attorney to draft for Cauncl1 consideratlon an amendment to
Resolution No. 4883(CCS) WhlCh would make Board/Commission appllcations
(or a portion of the appllcatlon) open to publlC review provlded that
advertlslng and the appllcatlon ltself so lndlcates. Cauncll directed
the Clty Clerk ta redeslqn the applicatlon form to permlt publlC reVlew
of the appllcant's expertise and statement of purpose whlle retalnlng
confldentlallty of address, telephone number and other personal lnforma-
tlon.
Sample appllcation forms were prepared for Council reVlew and comment.
Attached Exhlblt A proposes a revised appllcatlon form which removes
personal informatlon, makes the upper 8>," X 11" portlon open to publlC
review, and retains as confldentlal the bottom of the form contalning

and Cornrnlsslons as well as a notatlon ~n the ~nstruct~on sectlon
restr~ct~ng double membershlp. ThlS lS added pursuant to Councll
motlon at their meeting of November 22, 1983, when Council directed
staff to prepare a resolutlon affirrnlng the traditlon of not having
double membersh~p on Boards and Cornrnlsslons, notlng exceptlons for
Appeal Boards, Corporations, Bureaus, or speciflc approval by Councll
vote.

Recornrnendatlon
It is recommended that City Councll reVlew the proposed'application
form and resolutlon, suggest desired revis~ons, approve a revlsed
board and COmmlSSlon appllcation form for the City Clerk to dlstrlbute
to appllcants and adopt a resolution permltting publlc reVlew of
appllcatlons to Clty Boards and Cornrn~sslons submltted in the future.
