SUBJECT: Revised Board/Commission Application Form and
Resolution Amendment to Permit Public Review
Introduction
This report presents for Council review a revised
Board/Comm1ssion application form and a resolution amend1ng
Resolution No. 4883 (CCS) ( which sets forth procedures) to
provide for public review of applications.
Background
The City Council, at its meet1ng of August 23, 1983, directed the
City Attorney to draft for Counc~l considerat1on an amendment to
Resolution No. 4883 (CCs) which would make Board/Commission
applicat10ns (or a portion of the application) open to public
review, provided that advertising and the application itself so
indicates. Council directed the City Clerk to redesign the
application form to pennit public review of the app11cant's
expertise and statement of purpose while retain1ng
confidentiality of address, telephone number and other personal
~nformation .
Sample applicat10n forms were prepared for Council rev~ew and
comment. There was added to the proposed form a line for
applicants to indicate their current membersh1p on Boards and
Commissions as well as a notation in the 1nstruction section
restricting double membership.

This was added pursuant to Council motion at their meetlng of
November 22, 1983, when staff was directed to prepare a
resolution affirm1ng the tradition of not having double
membership on Boards and Commissions, noting exceptions for
Appeal Boards, Corporations, Bureaus, or spec iflC approval by
Councl1 vote.
The matter was considered at the City Council meeting of January
10, 1984, at which time a revised application form was considered
which returned ethnic, age and renter/owner information as
optional on the confidential portion. Further, 1t was requested
a.t that meeting to delete inquiry into marital status, number of
persons in household, and name of hlgh school and college, and to
add more lines for the applicant to fill in abilities which
qualify for service on the Commission as well as what the
applicant would hope to accomp11Sh as a member. The attached
revised Board/Commiss10n application form contains all of the
above suggestions, is in a form which permits the upper 8 1/2" by
II" portion to be open to public rev iew , and retains as
confidential the bottom portion of the application which contains
address, telephone number, age, renter/owner and ethnicity
information. The attached resolution provides that a portion of
the application shall be a public record.

Recommendation
It is recommended that City Council approve the attached Board/
Commission application form and adopt the resolut1on permitting
public review of the appropriate portion of app1icat1ons
subrn1tted in the future.
